Safety Requirements vs. Reliability Requirements: Why the Distinction Matters
Ask a room of systems engineers whether a requirement is a safety requirement or a reliability requirement, and you will often get confident answers that contradict each other. Both concern failure. Both appear in the same sections of requirements documents. Both get cited during hazard analysis reviews. But they are asking fundamentally different questions — and treating them as synonyms is one of the most common sources of verification gaps in safety-critical programs.
This article draws the boundary precisely, explains why it matters for design and verification, and shows how modern tooling can prevent the conflation from happening in the first place.
The Core Definitions
A safety requirement constrains the system to prevent a specific hazardous event from causing harm to people, the environment, or the broader mission. Safety requirements derive from hazard analysis. The chain is: identified hazard → assessed severity and exposure → determined risk level → requirement that either eliminates the hazard or controls it to an acceptable level. The requirement does not need to invoke probability at all; in many cases it is absolute. The system shall prevent simultaneous energization of both brake circuits is a safety requirement. It does not say “with 99.9% probability.” It says: this event cannot happen.
A reliability requirement specifies the probability that the system performs its intended function under defined conditions over a defined time interval without failure. It is a statistical statement about functional performance. The actuator shall demonstrate a mean time between failures (MTBF) of no less than 20,000 hours under operational profile X is a reliability requirement. It does not address what happens if the system does fail, or whether any particular failure mode causes harm.
The distinction is not semantic. It is structural. Safety requirements are rooted in consequence — what harm results from a specific failure path. Reliability requirements are rooted in frequency — how often does the system fail to perform its function. These are related quantities, but they are not the same quantity, and optimizing for one does not automatically satisfy the other.
How a Reliable System Can Be Unsafe
Consider an automotive power steering controller with an MTBF of 100,000 hours. By most reliability standards, that is an excellent number. But suppose the failure mode analysis reveals that when the controller does fail, it defaults to full left-lock steering input. The system is statistically reliable — it almost never fails. But when it fails, it causes a crash. That single failure mode, however rare, may be assigned ASIL D under ISO 26262 because the severity of the consequence is catastrophic and the controllability is low.
Reliability is high. Safety integrity is unacceptable. A requirements document that conflates the two would have accepted the reliability figure as evidence of safety adequacy. It is not.
This scenario plays out in aerospace, industrial control, and medical devices as well. In IEC 61508 terms, a SIL rating applies to a safety function — not to the system as a whole, and not to its statistical uptime. A Safety Instrumented Function can be assigned SIL 2 while the associated process equipment has conventional reliability requirements specified at much lower rigor. They coexist in the same system but live in different requirement spaces with different verification obligations.
How a Safe System Can Have Low Reliability
The converse is equally important. A system designed to be safe through functional redundancy, de-energized fail-safe states, and conservative operating limits may have relatively poor reliability from the user’s perspective — it trips, shuts down, or degrades gracefully far more often than a less conservative design. Nuclear safety systems, for example, are explicitly designed to go to a safe state on uncertainty. They may be highly interruptive to operations. The system is safe by design. It is not, by the same design, highly available.
Specifying both properties demands separate requirements. Availability targets (a reliability family metric) and safety integrity targets (a safety family metric) must be balanced against each other, and the tradeoff is a design decision that needs to be documented and traceable. Merging the two into a single requirement — “the system shall be reliable and safe” — obscures the tradeoff entirely and makes it impossible to verify either property independently.
How They Are Specified Differently

Safety requirements are derived from structured hazard analyses: Hazard Analysis and Risk Assessment (HARA) in automotive per ISO 26262, Preliminary Hazard Analysis (PHA) or System Hazard Analysis (SHA) in aerospace per MIL-STD-882 or ARP4761, or Layer of Protection Analysis (LOPA) in process industries per IEC 61511. The output is a set of safety goals or safety functions with assigned integrity levels.
ASIL (Automotive Safety Integrity Level) in ISO 26262 runs from A (lowest) to D (highest), assigned based on Severity, Exposure, and Controllability parameters from the HARA. An ASIL assignment says: this safety requirement must be implemented with design and verification rigor appropriate to this level. Hardware architectural metrics (SPFM, LFM, PMHF) must meet ASIL-specific thresholds.
SIL (Safety Integrity Level) under IEC 61508 and its domain derivatives (IEC 62061 for machinery, IEC 61511 for process) runs from 1 to 4. SIL is assigned to a safety function and carries both probability of failure on demand (PFD) and probability of dangerous failure per hour (PFH) targets, depending on whether the function is demand-mode or continuous-mode. SIL requirements specify what the safety function must achieve; they do not say the overall system must be reliable in the general sense.
A well-formed safety requirement looks like this:
SR-047 [ASIL C]: The brake-by-wire ECU shall detect loss of communication with the pedal sensor within 10 ms and apply a hydraulic fallback braking force of no less than 0.3 g within 50 ms of detection.
Note the specificity: the hazard scenario is implicit (loss of pedal signal while braking), the response is defined, timing constraints are given, and the ASIL tag is attached directly to the requirement.
Reliability requirement specification
Reliability requirements are derived from system reliability models — reliability block diagrams (RBDs), fault trees used for reliability (not just safety), or FMEA-based failure rate allocation. The inputs are operational profiles, mission durations, and system-level reliability or availability targets handed down from the customer or derived from the CONOPS.
A well-formed reliability requirement looks like this:
RR-012: The brake-by-wire ECU shall demonstrate a MTBF of no less than 50,000 hours under operational profile OP-3 (defined in Section 4.2), with a confidence level of 90% as demonstrated by accelerated life test per MIL-HDBK-217F.
Note what is different: there is no hazard reference, no ASIL tag, no response behavior specified. The requirement is about the statistical frequency of any functional failure, verified by test or analysis against a reliability model.
Both requirements may apply to the same physical unit. They have different derivations, different verification methods, and different responsible engineers. Keeping them separate is not bureaucratic overhead — it is the only way to ensure both get addressed.
What Goes Wrong When You Conflate Them
The failure modes of conflation are predictable:
Verification gaps. A requirement that says “the system shall be reliable and not cause harm” can be closed out by a reliability test that demonstrates acceptable MTBF, with no verification that specific failure modes are safe. The safety half of the requirement was never tested.
Design optimization errors. A designer told only to “maximize reliability” may add redundancy that increases component count and introduces common-cause failure modes that are actually less safe — trading improved MTBF for reduced single-fault tolerance. If safety and reliability goals had been specified separately, the design tradeoff would have been visible.
Certification non-compliance. ISO 26262 and IEC 61508 both require explicit safety requirement artifacts traceable to the hazard analysis. A requirements database full of “the system shall be reliable and safe” statements does not satisfy this traceability obligation, regardless of how much test evidence is attached.
Audit findings. Certification auditors and independent safety assessors look specifically for typed, attributed safety requirements with clear derivation chains. Untyped requirements that blend safety and reliability intent are a predictable source of audit findings and rework late in a program.
How Modern Tools Implement This Separation
Older document-based requirements tools — DOORS classic being the canonical example — enforce no typing discipline by default. A requirement is a row in a module. Whether it is a safety requirement, a reliability requirement, a performance requirement, or an interface requirement depends entirely on what the author wrote in the text field and whether the project has established naming conventions rigorous enough to enforce the distinction. Many have not.

Practical Starting Points
If your program is currently conflating safety and reliability requirements, the path forward does not require a full tool migration. Start with process:

Audit your requirement types. Pull every requirement that contains the words “safe,” “reliable,” “failure,” or “fault.” Classify each as safety-derived (traceable to a hazard), reliability-derived (traceable to a reliability model), or both. Requirements that legitimately belong in both categories should be split into two linked requirements, not left merged.

Tag before you trace. Every safety requirement should carry its ASIL or SIL level as an attribute before you build any traceability. Every reliability requirement should carry its allocated failure rate or MTBF target. Without these attributes, traceability matrices are decoration.

Require separate verification methods in the plan. Your verification and validation plan should explicitly identify which requirements are verified by hazard analysis cross-reference and architectural metric calculation (safety) and which are verified by reliability test, FMEA, or reliability block diagram analysis (reliability). If both methods appear in the same test entry, the entry is likely addressing a merged requirement that needs to be split.

Enforce the distinction at review gates. A requirement review that does not check for typed, attributed safety requirements against HARA outputs is not a safety review — it is a writing review.
The distinction between safety and reliability is foundational to safety-critical systems engineering. It is not a paperwork convention. It reflects two genuinely different properties of engineered systems, two different analytical traditions, and two different regulatory frameworks. A requirements process that cannot tell them apart cannot credibly claim to address either.
