How to Run a Requirements Review That Finds Real Problems
Requirements reviews have a reputation problem. Ask any senior systems engineer about the last formal review they attended, and you’ll hear one of two complaints: the meeting was two hours of debating comma placement, or the team waved everything through because the schedule was tight and nobody wanted to be the person who held the program. Either way, the real problems—missing constraints, untestable acceptance criteria, undocumented assumptions—made it into the baseline and came back later as expensive surprises.
This guide is for engineers who want reviews that actually work: structured enough to find systemic issues, efficient enough that people show up prepared, and disciplined enough to close without ambiguity. It covers preparation, checklist construction, the most common anti-patterns, how to write review comments that get acted on, and how to close a review cycle without letting open items drift.
Why Most Reviews Miss the Real Problems
The failure mode isn’t lack of expertise. Hardware and systems review teams typically contain experienced engineers who absolutely know what a bad requirement looks like. The failure mode is process structure that doesn’t direct that expertise toward the right questions.
Three structural problems cause this:
Reviews happen too late. When a requirements document arrives at a formal review with 300 line items, reviewers are implicitly under pressure to approve it. The sunk cost of the authoring effort, combined with schedule pressure, biases the room toward “conditional approval with comments” rather than substantive rethinking. Effective reviews happen iteratively, at the point when rework is still cheap.
Review inputs are unqualified. If every requirement in the package arrives with the same status—draft, under review—reviewers have no signal about which items the author is confident about, which are placeholders, and which have known open questions. Reviewers spend equal time on all of them, which means they spend too little time on the ones that actually need it.
Comments aren’t actionable. A review that produces a list of free-text observations with no ownership, no classification, and no resolution criteria doesn’t close. It produces a follow-up meeting that produces another list.
Fixing these three problems is the practical content of the rest of this guide.
Preparation: The Author’s Responsibility
The most important thing to understand about preparation is that it belongs to the author, not the reviewers. Reviewers should read the document before the meeting. Authors should do substantially more.
Before submitting a requirements package for review, the author should complete the following:
Annotate confidence and status explicitly. Every requirement should carry a status tag: STABLE, DRAFT, or OPEN QUESTION. Reviewers use this to allocate attention. OPEN QUESTION items signal “I need help here”—these should receive the most scrutiny. STABLE items signal “I’m confident in this; verify don’t reconstruct.”
Provide the parent context. Every child requirement should be accompanied by the stakeholder need or system requirement it derives from. If a reviewer can’t see why a requirement exists, they can’t evaluate whether it says the right thing. This is not the same as a full traceability matrix—it means a direct reference to the parent is visible in the review package.
Write the verification method for every requirement before review. This is the single most productive forcing function in requirements engineering. If you cannot write an acceptance test or inspection criteria for a requirement before the review, the requirement is not ready for review. Attempting to write the verification method will expose ambiguity, missing constraints, and circular specifications faster than any checklist will.
Prepare a delta summary if this is a revision. For reviews of revised documents, provide a structured summary of what changed and why. Reviewers should not have to diff documents manually. Changes fall into three categories: stakeholder-driven changes, error corrections, and clarifications. Separating these helps reviewers understand what needs substantive re-evaluation versus what is editorial cleanup.
The Review Checklist: Making Expertise Repeatable
A checklist doesn’t replace engineering judgment—it channels it. The goal is to ensure that every reviewer evaluates every requirement against the same set of fundamental questions, not just the questions that happen to come to mind in the moment.
The following checklist is organized into four categories. It is not exhaustive; adapt it to your domain and project phase.
Completeness
- Does the requirement state what the system shall do, not how it shall do it (unless the how is genuinely a constraint)?
- Are all applicable conditions, modes, and environments identified?
- Are all undefined terms either defined in the glossary or defined inline?
- Is there a reference to the parent need or system requirement?
Correctness and Consistency
- Is the requirement consistent with its parent? Does it implement the parent fully without adding scope not traceable to a stakeholder need?
- Are numerical values accompanied by tolerances and units?
- Are any quantities that appear in multiple requirements consistent with each other?
- Does the requirement avoid conflicting with other requirements in the document or in referenced documents?
Verifiability
- Can you write an acceptance test, analysis procedure, or inspection criteria for this requirement today? (If no: the requirement is not ready.)
- Does the requirement avoid subjective qualifiers—“sufficient,” “adequate,” “user-friendly”—unless they are quantified?
- Is the pass/fail condition unambiguous?
Implementability and Risk
- Is the requirement technically feasible given the current design baseline and schedule?
- Are there requirements that push against the edge of known supplier capability? Have those risks been documented?
- Does any requirement duplicate or conflict with a standard, regulation, or interface document? Is that conflict flagged?
Distribute this checklist to reviewers with the review package. Ask reviewers to mark each item for each requirement. This creates a structured record of what was evaluated—not just what was found—which is valuable when the question later arises of whether an issue was reviewed and missed or simply never reviewed.
Common Anti-Patterns
These appear in real review sessions across programs. Naming them helps teams recognize and stop them.
The formatting spiral. The review consumes its first forty-five minutes on punctuation, numbering conventions, and template compliance. These are real issues but they are not review issues—they are pre-review issues. Authors should run documentation standards checks before submitting. If the document fails basic formatting, return it to the author; don’t burn review time on it.
The design solution disguised as a requirement. “The unit shall use a 32-bit ARM Cortex-M4 processor” is a design decision, not a system requirement, unless there is a documented reason the architecture is constrained. Reviews should challenge these explicitly, not because the design decision is necessarily wrong, but because premature design lock-in in requirements limits supplier and implementation options unnecessarily.
The requirement that says the system shall try. “The system shall attempt to complete the transaction within 200ms.” The word “attempt” removes verifiability. Either the requirement is met or it isn’t. This language appears when authors are uncertain about performance margins and are trying to hedge. The review should surface the uncertainty, not accept the hedge.
The passive reviewer. In large review meetings, individual reviewers assume others are covering the hard questions. Assign specific reviewers to specific requirement sections with documented ownership. Generic review responsibility is effectively no responsibility.
Approval as a social act. When a reviewer knows the author personally, has worked on the program for two years, and is facing a gated milestone next week, approving with minor comments is the path of least resistance. Reviews need a norm—not just a rule, but an actively maintained norm—that substantive technical disagreement is expected and valued. Program managers who penalize engineers for raising real issues in reviews are building technical debt.
How to Write Review Comments That Get Acted On
A good review comment has four elements: a precise location reference, a classification, a clear description of the problem, and a suggested resolution or question.
Location reference. Requirement ID and the specific clause or sentence. Not “Section 3” or “the power requirements.” Example: REQ-PWR-047, clause 2.
Classification. Use three categories:
ACTION: The requirement must change. The comment describes what is wrong and what correct looks like.QUESTION: The reviewer does not have enough information to evaluate the requirement. The comment specifies what information is needed.DEFER: The reviewer believes the requirement is out of scope for this review cycle or depends on an upstream decision. The comment explains the dependency.
Problem description. State what is wrong, not just that something is wrong. “This requirement is ambiguous” is not useful. “This requirement does not specify whether the 200ms latency budget applies at the component interface or end-to-end from user input to system response” is useful.
Suggested resolution. Even if it’s imperfect. A reviewer who identifies a problem and proposes a candidate fix gives the author something to work with. Authors can reject the proposed fix while accepting that the problem is real.
Comments filed without a classification or a location reference should be returned to the reviewer for correction before they enter the official review record. Unstructured comments create ambiguity about what was actually decided.
Automated First-Pass Checking: Clearing the Noise
Before human reviewers spend time on a requirements package, a substantial fraction of common issues can be detected automatically: undefined terms, missing verification methods, passive voice constructions that hide the system subject, requirements with no parent link, duplicate IDs, and numerical values without units.
Running automated quality checks before review does two things. First, it returns low-quality documents to authors before they consume review time. Second, it means that by the time human reviewers read the document, the easy problems are already resolved—which means reviewer attention goes to the hard problems that automated tools cannot detect: missing constraints, incorrect allocations, conflicting assumptions, and requirements that are technically legal but operationally wrong.

Closing the Review Efficiently
A review that doesn’t close is worse than no review—it creates an ambiguous baseline and an unresolved action list that accumulates.
Closure requires three things:
Every comment has an owner and a due date. “The team” does not own a comment. A named engineer owns it. Due dates are specific. If a comment requires an upstream stakeholder decision before it can be resolved, that dependency is explicit and a follow-up trigger exists.
Resolution criteria are agreed before the review ends. For ACTION items: what does the corrected requirement need to say for the comment to be closed? For QUESTION items: who answers the question, and by when? For DEFER items: what event or decision triggers reconsideration?
A re-review scope is defined for revised documents. When comments result in requirement changes, reviewers should receive a targeted re-review package covering only changed requirements plus any requirements affected by the change. Full document re-review on every revision cycle is expensive and produces diminishing returns.
The review closes when all ACTION and QUESTION items are resolved and the resolution has been verified by the reviewer who raised the comment, or by the review chair if the original reviewer is unavailable. DEFER items move to a tracked open items list with a defined trigger for re-evaluation.
Practical Starting Points
If your current review process needs improvement, start with the three highest-leverage changes:
Require verification methods before submission. If a requirement doesn’t have a documented verification method, it doesn’t enter review. This single gate will cause authors to catch their own ambiguities before reviewers see them.
Implement the three-category comment classification. Every comment gets ACTION, QUESTION, or DEFER. This alone will shorten review closure cycles by eliminating the ambiguous middle ground where comments technically exist but nobody knows what to do with them.
Run automated quality checks as a submission gate. Whether through a dedicated tool or a structured spreadsheet-based checklist run by the author, automated pre-screening ensures reviewers read qualified documents. Human expertise is the scarce resource in a requirements review. Protect it.
Requirements reviews are not a compliance ritual. They are a technical risk management activity. The infrastructure around them—preparation discipline, structured checklists, actionable comments, defined closure—determines whether they function that way in practice.
